When you first come to our office if it is not generally for an emergency we like to do a comprehensive examination of your dental situation. We want to know you as a person to enable us to better meet your expectations.
What we will need at the time of your appointment is information regarding your dental/medical insurance. Our goal is to maximize your insurance and minimize our patients out of pocket expense.
At the first appointment we will be discussing your medical history. If you take a lengthy list of medicines bring the list with you so that we can accurately update your information.
Please assist us by providing the following information at the time of your consultation:
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e,. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) .
If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
